Your First Visit

arrival

If it is your first appointment, please arrive 5 minutes earlier to complete a new patients form. Appointments run for 30 minutes and include a diagnostic consultation as well as treatment. Each of our physiotherapists see one patient at a time.

There is plenty of free parking available right in front of our clinic. 

sms reminders

 

On the day before your appointment, an SMS will be sent to your mobile phone number to remind you of your appointment time. If you are unable to make it your appointment, Please call 02 6242 5050 to reschedule or cancel the appointment.

HICAPS

 

Please bring along your Private Health Insurance card. We can scan your card to get the rebate from your fund, so that you will only need to pay the gap.

Pension Concession Card Holder 

 

We offer a discount for pension concession card holders. Please let the receptionist know during your visit.

 

Enhanced Primary Care Plans

 

The cost of the Enhanced Primary Care treatment can be claimed back on Medicare. We require full payment at the time of consultation. Your receptionist will then process your claim with Medicare. Your benefit amount will differ from person to person (this amount will depend on your income). The benefit amount will go straight into your nominated bank account that you have registered with Medicare in 1-3 business days. Please bring along your EPC referral (from your GP) to your first physiotherapy appointment.

 

what to wear

 

Please wear loose and comfortable clothing such as track pants, shorts and t-shirt to your appointment.

 

referrals 

A doctors referral is NOT required if you are a private patient (i.e. not claiming on workers compensation or a third party). However, if you do have referral, scan or XRAY, please bring it in with you and give it to the receptionist to add to your file. They will pass it along to your treating physiotherapist when they are done.

 

X-rays and diagnostic scans

 

Please bring along any relevant x-rays and scans or online usernames/passwords to your physiotherapy appointment.

pilates 

 

If you have never been to our clinic before and you would like to join our pilates class, you will need to come in for an initial assessment so that we can cater a pilates program to you. Our sessions are taught by a registered physiotherapist, Alana Coombe. They are held at Forde Community Centre.

WHAT TO BRING: Water and a small towel. We will supply a mat.

WHAT TO WEAR: Loose comfortable fitting clothes
